Procedure for Confirmation of Enrollment

FOR QUALIFIED AND ACCEPTED ON-WAITING LIST APPLICANTS:

1. Visit your USTET results portal at https://ustet.ust.edu.ph/results/ to download your Confirmation of Enrollment form and Conditional Admission Contract.

Note: In view of the COVID-19 situation, the Bureau of Immigration allows the enrollment of foreign students pending the processing of student visa or study permit.

2. View the Faculty of Engineering AVP

3. Download NSTP/ROTC Primer at [NSTP-Primer-2020]

4. Accomplish downloaded forms and prepare scanned copy of each of these forms, and of the following additional requirements:
    a. Form 138
    b. Certificate of Good Moral Character from SHS
    c. 2 x 2 picture (Colored)

5. Click your respective Program link (Google Form) for Confirmation of Enrollment and uploading of scanned documents. 

    Deadline is on May 29, 2020

   Google Form for Civil Engineering
   Google Form for Chemical Engineering 
   Google Form for Mechanical Engineering
   Google Form for Electrical Engineering
   Google Form for Electronics Engineering
   Google Form for Industrial Engineering

6. Wait for the email confirmation from the Faculty of Engineering. (Allow for 5 working days after sending your documents.) 

     Note: You will be notified through this email confirmation on the procedure of enrollment.

7. After 3 working days from being confirmed by the Faculty of Engineering, login at https://pds.ust.edu.ph to accomplish the Personal Data Sheet (PDS) online.

8. Enrollment is on June 8-11, 2020.

Procedure for Confirmation of Enrollment

FOR QUALIFIED AND ACCEPTED ON-WAITING LIST APPLICANTS:

1. Visit your USTET results portal at https://ustet.ust.edu.ph/results/ to download your Confirmation of Enrollment form and Conditional Admission Contract.

2. Read the prospectus of the BA program applied for at:

3. Download NSTP/ROTC Primer at [NSTP-Primer-2020]

4. Read and accomplish the applicable forms and prepare scanned copies of the required credentials:
   a. PDF copy of Confirmation of Enrollment Form
   b. JPEG copy of 2x2 picture of the applicant
   c. PDF copy of Final FORM 138 (High School Report Card)
   d. PDF copy of Certificate of Good Moral Character
   e. PDF copy of the CONDITIONAL ADMISSION CONTRACT (CAC) (in lieu of the original Form 138 and the GMC certificate).
       The applicant and his/her parent must affix their signatures over their printed names in the conditional admission contract.

Note: If the report card and/or GMC is not available, accomplish the CAC.
            If the report card reflects the applicant’s grades only up to the 3rd Quarter, accomplish the CAC.

5. Upload clear copies of the scanned documents through the Google Form link for your degree program. Deadline is on May 27, 2020.

6. Wait for the email confirmation from the Faculty of Arts and Letters. Please wait for at least five working days after the submission of the required documents.

7. After 3 working days from being confirmed by the Faculty of Arts and Letters, login at https://pds.ust.edu.ph to accomplish the Personal Data Sheet (PDS) online.

8. Enrollment is on June 8-11, 2020.

COLLEGE OF NURSING

Procedure for Confirmation of Enrollment

FOR QUALIFIED AND ACCEPTED ON-WAITING LIST APPLICANTS:

1. Visit your USTET results portal at https://ustet.ust.edu.ph/results/ to download your Confirmation of Enrollment form and Conditional Admission Contract.

Note: In view of the COVID-19 situation, the Bureau of Immigration allows the enrollment of foreign students pending the processing of student visa or study permit.

2. View the orientation video of the UST College of Nursing and read the Frequently Asked Questions regarding the policies for admission. 

3. Download NSTP/ROTC Primer at [NSTP-Primer-2020]

4. Accomplish the downloaded form and scan the following additional requirements:
   4.1 Accomplished downloadable Confirmation of Enrolment Form (from their USTET portal)
   4.2 Signed scanned copy of the College of Nursing Contract (submitted online after the online orientation; deadline date
         and details to be announced.) The link to this document will be given to you during the online orientation.
   4.3 Scanned copy of Form 138 (Senior High School Report Card)
   4.4 Scanned copy of certificate of Good moral Character
   4.5 Accomplished Waiver for those who cannot submit a copy of their report card (Form 138) prior to enrolment
   4.6 Alien Certification of Registration (ACR) for resident or immigrant aliens
   4.7 Passport/student visa for non-resident aliens
   4.8 2x2 colored ID picture (electronic copy, preferably)

5. Upload your scanned documents to the  Nursing Google Form. Deadline is on May 25, 2020.
    Kindly LABEL each required document with the following: Last name _ First name _ type of document

    Google Form for College of Nursing 

    AFTER completing the google forms, the college will email you your schedule and online orientation link.

6. Wait for the email confirmation from the College of Nursing. (Allow for 5 working days after sending your documents.)

7. After 3 working days from being confirmed by the College of Nursing, login at https://pds.ust.edu.ph to accomplish the Personal Data Sheet          (PDS) online.

8. Attend the SCHEDULED ONLINE confirmation/ orientation to be conducted by the College of Nursing on May 26, 27 and 28, 2020. 
    Your schedule and link will be emailed to you after completing the google form.

9. Enrollment is on June 8-11, 2020.

Procedure for Confirmation of Enrollment

FOR QUALIFIED APPLICANTS:

1. Download and read the Program Policies of the Faculty of Medicine and Surgery.
    Link:  Doctor of Medicine Program
  Video:  UST Medicine at 150

2. Accomplish the Personal Date Sheet that the Office for Admission sent to your email address.
    Have it scanned, and together with a photo of your reservation deposit slip, 
    send them to our email address: medicine.admission@ust.edu.ph

3. You will receive an acknowledgement mail with a google form that you should accomplish.
    We hope to receive your response not later than 23 May 2020.

4. Wait for the email confirmation from the FMS Admission Committee within 5 working days.

5. Starting 30 May 2020, login at https://pds.ust.edu.ph/ to accomplish the Personal Date Sheet online
    and have a screenshot of the Acknowledgement Slip.

6. Open MyUSTe Student Portal using the student number from the PDS Online and date of birth,
    and go to the Enrollment tab, Online Enrollment (https://myuste.ust.edu.ph/student/)

7. Proceed with online enrollment by agreeing to the Office for Student Affairs’ Enrollment Conforme.

8. Proceed with online assessment. Pay through any Metrobank or Bank of the Philippine Islands branches.

9. Check your MyUSTe Student Portal and download your registration form.

10. Enrollment is on 08-11 June 2020.

11. Once the community quarantine is lifted, prepare the ORIGINAL documents listed under
      PRE-ENROLLMENT REQUIREMENTS for submission.

Procedure for Confirmation of Enrollment

FOR QUALIFIED AND ACCEPTED ON-WAITING LIST APPLICANTS:

1. Please send a copy of your reservation deposit slip and scanned Personal Data Sheet: 
    bs.basic-human-studies.admission@ust.edu.ph

2. You will receive an acknowledgement mail with a google form that you should accomplish.
    We hope to receive your response not later than 30 May 2020.

3. Wait for the email confirmation from the FMS Admission Committee within 5 working days.

4. Starting 08 June 2020, login at https://pds.ust.edu.ph/ to accomplish the Personal Date Sheet online 
    and have a screenshot of the Acknowledgement Slip.

5. Open MyUSTe Student Portal using the student number from the PDS Online and date of birth,
    and go to the Enrollment tab, Online Enrollment (https://myuste.ust.edu.ph/student/)

6. Proceed with online enrollment by agreeing to the Office for Student Affairs’ Enrollment Conforme.

7. Proceed with online assessment. Pay through any Metrobank or Bank of the Philippine Islands branches.

9. Enrollment is on 18-19 June 2020.
